MyChart® Patient Portal Terms and Conditions of Use

Lexington Medical Center is pleased to provide the web site MyChart® to allow patients and appointed patient representatives, called “proxies,” to access certain portions of the patient’s medical record.

Never use MyChart for emergency or other urgent medical issues. If you are experiencing a medical emergency or need urgent medical care, immediately call 911, go to your local emergency room, and/or call your physician.

MyChart is not your complete medical record. MyChart provides you with access to some (but not all) of the information contained in your electronic medical record. As a result, you should not rely on information accessed via MyChart as complete. If you would like to request a copy of your record, contact your primary physician clinic or the Health Information Management department at Lexington Medical Center at (803) 791-2264.

Please note that not all test results are available in MyChart. It is your sole responsibility to follow up with your physician to request, review, and discuss any tests or test results, and to inquire with your physician if you do not see results of tests in MyChart that you believe have been done.

These MyChart Terms and Conditions govern your use of MyChart and this web site, describe how Lexington Medical Center handle information communicated via the Internet, and outline our practices and our respect for your privacy.

If you do not wish to be bound by these terms and conditions, do not access or use MyChart. The use of MyChart is completely voluntary and is not required to receive health care from Lexington Medical Center.

I. Eligibility

Individuals must be age 16 or older to request a personal MyChart Account.

Parents or guardians of minor patients, patient representatives, or other individuals may request access to a patient’s MyChart Account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement.

II. Proxy Access

You may request proxy access to the LMC MyChart Account of another individual, or you may request another individual have access your LMC MyChart Account, by submitting the appropriate Proxy Application and Authorization form to the patient’s primary physician clinic or the Health Information Management department at Lexington Medical Center (2720 Sunset Blvd., West Columbia, SC 29169). A person receiving proxy access must establish his or her own MyChart Account, and he or she is required to adhere to these Terms and Conditions. Proxy Application and Authorization forms are available from the Health Information Management Department of Lexington Medical Center or the patient’s primary physician clinic.

Proxy for Minor Patients (Birth – 15 years)

For access to an LMC MyChart Account of children under age 16, parents or guardians should complete the “Proxy for Minor Application.” A parent or guardian’s proxy access to a minor patient’s MyChart Account will automatically terminate on the day prior to the child’s sixteenth birthday.

Proxy for Adult Patients and Minor Patients age 16 and over

For access to the LMC MyChart Account of an adult patient or a minor patient ages 16-17, the patient and the proposed proxy complete the “Proxy for Adult Application.” Proxy access to an adult patient’s MyChart Account will automatically terminate three years after establishing proxy access.

Proxy for Adult Patients pursuant to Guardianship or Healthcare Power of Attorney

For access to the LMC MyChart Account of an adult patient over the age of 16 who is subject to a guardianship or healthcare power of attorney, the proposed proxy should complete the “Proxy for Adult Application Requiring Legal Documentation.” The proposed proxy must provide copies of necessary court orders and documentation verifying the relationship and effectiveness of the appointment. If the proxy’s legal relationship to the patient changes, the proxy must notify Lexington Medical Center immediately. Proxy access to the MyChart Account of an adult patient over the age of 16 who is subject to a guardianship or healthcare power of attorney will automatically terminate one year after establishing proxy access.

III. E-mail Privacy

Users of MyChart will be notified by e-mail when there is a new message or new medical information to be viewed on MyChart. This means that any person with access to your e-mail will be able to see this notification. This could include your spouse, child, employer, or anyone else who can access your email account. Although no private medical information will be sent, the notification that new medical information is available in MyChart may be information that you would not want others to know. It is your sole responsibility to carefully choose the email address you provide for use with your MyChart Account and to take all necessary precautions to prevent others from gaining unwanted access to these notices, your email, and other private, confidential or sensitive information.

Certain portions of MyChart permit you to securely exchange electronic messages and other information with Lexington Medical Center providers. You agree that you will not use MyChart, including its messaging functionality, for emergency or other urgent healthcare needs, or as a substitute for actual medical advice and care by a physician. You also agree that you will use the messaging function appropriately, and not send excessive, unnecessary, or inappropriate messages through MyChart.

All electronic messages you send through MyChart will become part of your Lexington Medical Center medical record and may be used or disclosed as provided in this Agreement and the Notice of Privacy Practices. Any messages you send must be about your healthcare and medical conditions, or those patients for whom you have proxy access, and not those of a friend, family member, or any other person.

Do not use a MyChart message if you want to communicate sensitive information directly with an individual practitioner. Your MyChart messages may not be delivered to your individual physician but instead may be sent to appropriate staff members for processing and response. Do not include private, confidential, or sensitive information that you do not want others to see in your messages.

It is your responsibility to ensure that complete and accurate information is provided. Any response to such communication will be based solely on the information provided.

Lexington Medical Center will respond to your message in as timely a manner as is reasonably possible. It is your sole responsibility to monitor your account for messages and other information and to respond and /or act (or not) upon them in a timely and appropriate manner.

IV. Security and Confidentiality

Lexington Medical Center will afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by Lexington Medical Center in any other way. Lexington Medical Center is committed to protecting the confidentiality of your medical information. Employees’ access and ability to enter or view information is based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. Lexington Medical Center will identify the records released and note the time and date of access each time you access MyChart. Lexington Medical Center has taken steps to make all information received online as secure as possible against unauthorized access and use. Lexington Medical Center is not responsible if you share with others your medical information that you print from MyChart.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Your activities in MyChart may be tracked by computer audit trails and the entries that you make may become part of your electronic health record.

Lexington Medical Center will make every effort to assure that MyChart is always working properly, but there may be times when the system is shut down. Lexington Medical Center is not to blame for any problems that may occur from being unable to access MyChart.

Notices of Privacy Practices applicable to you are available from your primary physician’s office or from These notices explain how health information about you may be used and disclosed, and your rights with respect to your health information.

V. Modification of the Terms and Conditions

Lexington Medical Center may modify these Terms and Conditions at any time. Any modifications made to these Terms and Conditions will be effective immediately upon your agreement to such modified terms or your continued use of MyChart (whichever occurs first). Any aspect of MyChart may be changed, supplemented, deleted, or updated with or without notice at the sole discretion of Lexington Medical Center. You will be able to see any modifications when you log-in to My Chart.

VI. Additional Requirements and Restrictions

When using MyChart, you agree that you shall: comply with these Terms and Conditions; comply with the law; keep your User ID and account password strictly confidential; and promptly notify us of any unauthorized use of your MyChart Account or password - or if you learn of any other security breach related to MyChart.

In addition to other restrictions set forth in these Terms and Conditions, when using MyChart, you agree that you shall not: impersonate any other person; use any portion of MyChart as a destination linked from any unsolicited bulk messages or unsolicited commercial messages ("spam"); use any automated process or service (such as a BOT, a spider, periodic caching of information, or "meta-searching") to access and/or use MyChart; use any unauthorized means to modify or reroute, or attempt to modify or reroute, MyChart; or damage, disable, overburden, or impair MyChart (or the network(s) connected to MyChart) or interfere with anyone's use of MyChart).

Lexington Medical Center reserves the right, at its sole discretion, to discontinue your MyChart Account and use of MyChart. The following is a list of reasons which may result in deactivation of a user’s MyChart Account. The list is not exclusive and each circumstance will be evaluated individually.

  • Five (5) failed login attempts to access MyChart (You will need to contact [803-791-2300] for assistance in unlocking the account);
  • Inappropriate use of communication function with Lexington Medical Center, such as excessive, unnecessary, or inappropriate content;
  • Abuse of proxy access;
  • Pending legal issues with patient or direct family;
  • Inability to use features without constant assistance; or
  • Fourteen (14) months of MyChart Account inactivity.

    You will be notified by email if your MyChart Account and access is discontinued.

    VII. Medical Disclaimer

    All health and health-related information in MyChart is intended to be general in nature, provided solely for patient convenience, and is not a substitute for a visit to a licensed healthcare provider. Please see your doctor, not MyChart, for all medical advice and care. MyChart constitutes neither the practice of any medical, nursing, or other health profession nor the practice of health care or any other professional advice, diagnosis, or treatment. The information in MyChart is not your complete, official medical record, nor should it be relied on in any way for medical decision-making or to suggest a course of treatment for you or anyone else. If you have or suspect that you have a medical problem or condition, call your physician. Never disregard medical advice or delay in seeking it because of something you read in MyChart.

    NEVER USE MYCHART FOR EMERGENCY OR OTHER URGENT MEDICAL ISSUES. If you are experiencing a medical emergency or need urgent medical care, immediately call 911, go to an emergency room, and/or call your doctor.

    VIII. Disclaimer

    I understand that Lexington Medical Center takes no responsibility for and disclaims any and all liability arising from any inaccuracies or defects in software, communication lines, virtual private network, the internet or my internet service provider (ISP), access system, computer hardware or software, or any other service or device that I use to access MyChart. I understand that Lexington Medical Center has no responsibility for and disclaims any and all liability arising from any possible loss of data or viruses or any other contamination or destructive software or programs or code arising from or in connection with my access or use of MyChart. I understand that MyChart may not be available to me at all times due to system failures, back-up procedures, maintenance, or other causes. Access is provided on an "as-is, as-available" basis and Lexington Medical Center does not guarantee that I will be able to access MyChart at any particular time. During times when MyChart is unavailable, other communication methods (e.g., telephone) should be used to contact my Lexington Medical Center care provider.

    MyChart, its content and all site-related services are provided "as is," with all faults, with no representations or warranties of any kind, either expressed or implied, including, but not limited to, the implied warranties of merchantability, fitness for a particular purpose or non-infringement. You assume total responsibility and risk for your use of MyChart, all site-related services, and third party web sites. No oral or written information or advice given by Lexington Medical Center or its authorized representatives shall create a warranty of any kind. Any references to specific products or services on MyChart do not constitute or imply a recommendation or endorsement by Lexington Medical Center unless specifically stated otherwise.

    Lexington Medical Center has no responsibility for and disclaims any and all liability to you or any third-party, arising from or in any way related to, whether directly or indirectly, MyChart confidentiality, privacy, or security failures due in whole or in part to any act or omission by you or any third party, including, without limitation, not logging out of an active session; lost or stolen user information; use of your MyChart account from a publicly available computer or internet kiosk (e.G., a computer in a public library or hotel business center); downloading, printing, publishing, reproducing, transcribing, storing, transmitting in any form or by any means, redistributing, or selling information from my chart; user computer settings or installed software products that may compromise information security; failure to maintain anti-virus software or to regularly apply software updates on the user's computer; or any other acts, omissions, events, or circumstances outside of Lexington Medical Center's control.

    IX. Limitation of Liability; Choice of Law

    Lexington Medical Center and its affiliates, suppliers, and other third parties mentioned or linked to or on MyChart are neither responsible nor liable for any direct, indirect, incidental, consequential, special, exemplary, punitive, or other damages (including, without limitation, those resulting from lost profits, lost data, or business interruption) arising out of or relating in any way to MyChart, site-related services and products, content or information contained within MyChart, and/or any third party web site, whether based on warranty, contract, tort, or any other legal theory and whether or not advised of the possibility of such damages. Your sole remedy for dissatisfaction with MyChart, site-related services, and/or third party web sites is to stop using the web site and/or those services. Applicable law may not allow the exclusion or limitation of incidental or consequential damages, so the above limitation or exclusion may not apply to you. These terms and conditions are governed by South Carolina law without regard to its principles of conflicts of law.

    X. Copyrights

    Except as otherwise indicated, all content on MyChart, including text, graphics, logos, button icons, photos, images, forms, audio, video, questionnaires, and software, is the property of Lexington Medical Center or its licensors and is protected by United States and international copyright laws. Lexington Medical Center allows you to view or download a single copy of the material on MyChart solely for your personal, noncommercial use.

    The compilation of all content on MyChart is the exclusive property of Lexington Medical Center and is protected by United States and international copyright laws. Unless specifically authorized in writing by Lexington Medical Center, any use of these materials, or of any materials contributed to MyChart by entities other than Lexington Medical Center, on any other Web site or networked computer environment for any purpose is prohibited.

    Any rights not expressly granted by these Terms and Conditions or any applicable end-user license agreements are reserved by Lexington Medical Center. Content and features are subject to change or termination without notice is the editorial discretion of Lexington Medical Center.
